Eligibility Criteria to Get Kalaignar Kapitu Thittam Card

  • Applicant must be a resident of Tamil Nadu 
  • Applicant’s family name must be present on the family ration card 
  • Annual income of the family should be less than INR 1,20,000

The family here includes the eligible member, legal spouse, children and dependent parents. 

Exclusions

  • Non-residents of Tamil Nadu
  • Members not listed on the ration card
  • Earning more than the mentioned income 
  • Migrants from other Indian states who lack a migration certificate from the appropriate authority
  • Non-Sri Lankan refugees who do not possess proof of citizenship 
  • Orphans and rescued girl children who are not associated with any registered or unregistered organisation

Application Process of the CM Health Card

The application to gain a CM Health Card can be done by the following method: 

  • Get the income certificate from the Village Administrative Officer or Revenue Authorities 
  • Head to the CMCHIS portal and download the CMCHIS enrolment form
  • Fill it carefully and accurately 
  • Head to the enrollment centre with all the necessary documents and the form 
  • Provide the documents to the kiosk operator to begin the verification process 
  • Provide the biometric details like eye scan, fingerprints and photograph 
  • An e-card will be issued after the verification of the stated details
